FORMS REQUIRED TO BE SUBMITTED TO THE DEPARTMENT BEFORE THE INTERNSHIP (MUST INTERNSHIP)

The following three documents must be submitted to the departmental internship coordinator by June 10, 2024, at the latest.

1. Students who will undertake an internship must fill out the following form to determine the place and date of the internship and get it signed by the Head of the Department.

 

2. Students who will undertake an internship must fill out the following document for their insurance and get it approved by the Department Head.

 

3. Students who will undertake an internship must also submit the “Eligibility Certificate” obtained from the Health Provision and Activation System (SPAS) along with the Student Insurance Entry Form to the department. Link: https://www.turkiye.gov.tr/spas-mustahaklik-sorgulama?belge=Goster

Students who will undertake a voluntary internship must submit the above three documents along with a petition addressed to the Department Head requesting the coverage of their voluntary internship insurance.

 

FORMS THAT CAN BE SUBMITTED TO THE ORGANIZATION WHERE YOU WILL DO YOUR INTERNSHIP BEFORE THE INTERNSHIP (MANDATORY INTERNSHIP)

The organizations where you will do your internship may request a document indicating that your internship is mandatory or that your internship insurance will be covered by the school. In this case, you can get the appropriate one of the following three forms approved by the Department Head and submit it to the organization (according to the organization’s request).

INTERNSHIP AGREEMENT

(It is not mandatory for all students doing a mandatory internship to bring the internship agreement. Prepare it if the organization requests an agreement from you.)

 

FORMS REQUIRED TO BE SUBMITTED TO THE DEPARTMENT AFTER THE INTERNSHIP (MANDATORY INTERNSHIP)

Students who will undertake an internship must click on the following link to download the document. Afterward, they must read the information in this document, fill it out accordingly, and submit it to the Department Head as soon as possible after the internship ends (If they are working for a fee, they must also fill out the “Information on Enterprise and Mandatory Internship” part of the document and bring the original bank receipt showing the payment of the fee.)

All forms and petitions must be written and signed with a blue pen, and the signed versions must not be photocopied. Otherwise, your documents will not be accepted.
